Every patient pays an affordable, flat monthly fee to become a member.

Unlimited Visits

Unlimited Office Visits with any of our providers

Procedures

Ear Lavage, Laceration Repair, I&D and EKG

Yearly Routine Lab Screening

CBC, CMP, A1C, Thyroid Panel, Lipid Panel and Vitamin D

Men > 20 Plan Includes:

Yearly PSA and Testosterone Lab Screening

Women > 21 Plan Includes:

Yearly PAP Smear

Age Groups & Cost

$100 enrollment fee per person ($50 for each additional spouse or child)

5-19 Years Old

$20/Month

20-39 Years Old

$40/Month

40-59 Years Old

$60/Month

>60 Years Old

$80/Month

Corporate Plan

$40/Month Per Employee with a minimum of 10 employees

Company wide $500 enrollment fee

Payment

Billing

Charges are billed on the day of enrollment, and on the 1st day of each month prior to services being rendered with a 12 month contract

Discounts

A 15% discount will be applied if total payment is received up front. Equals 1.5 free months

Insurance

We cannot accept HSA or use any insurance for Direct Primary Care

Re-Enrollment

To prevent frequent or repeat membership cancellations, a $450 re-enrollment fee will be charged
